Commissioning is not notarizing—it means:
- The deponent (person signing) declares that the contents of a document (like an affidavit or statutory declaration) are true
- This declaration is made under oath or affirmation before a Commissioner
- The Commissioner signs, dates, and stamps the document to confirm the declaration
Common Documents that Require Commissioning
- Affidavits (e.g., for court, insurance, small claims)
- Statutory Declarations (e.g., name change, common-law status, residency)
- Lost/Stolen ID declarations
- Government forms (immigration, CRA, Service Canada)
The Commissioning Process (Step-by-Step)
- Review the Document
Ensure it’s the correct form and requires commissioning (not notarization). - Verify Identity
Ask for valid government-issued photo ID (driver’s license, passport, PR card, etc.). - Administer the Oath or Affirmation
- Oath: “Do you swear that the contents of this document are true, so help you God?”
- Affirmation: “Do you solemnly affirm that the contents of this document are true?”
- Witness the Signature
The person must sign in front of you.
You may initial any corrections if necessary. - Complete the Jurat
Fill in the commissioning details:
“Sworn (or affirmed) before me at [City], in the Province of Ontario, this ___ day of [Month], [Year]” - Sign, Date, and Stamp
Apply your commissioner stamp/seal and license number if required.
Keep a record in your logbook (optional but recommended). - Provide a Copy or Receipt
