A 25-year-old man was killed and four others injured in the accident on Friday, July 26 on the 407 at Lakeridge Road in Whitby.
The OPP at the time reported an eastbound vehicle crossed over into the westbound lanes and struck another vehicle.
A Colborne woman was charged on Friday, Nov. 22 with one count of careless driving causing death and two counts of careless driving causing bodily harm.
